Key Responsibilities\n

• Conduct comprehensive assessments to identify patient needs and tailor individualized treatment plans.

• Facilitate clear and ongoing communication between supervisors, direct care staff, family members, and other stakeholders to monitor and share patients' progress.

• Provide a structured schedule of appointments, home visits, and observations to sum up work advancement and ensure targeted support.
\n
• Coordinate necessary home visits for in-depth patient understanding, which includes chart reviews, staff interviews, and data collection and analysis.

• Guarantee that the implementation of treatment plans is effective, aligning with the organizational goals of improving patient outcomes.

• Participate in Interdisciplinary Team Meetings to discuss multifaceted patient cases and enhance collaborative care approaches.

• Create and maintain detailed documentation through monthly and quarterly progress notes reflecting patients' development.
\n
• Organize and lead training sessions for staff to ensure competency in implementing programs and maintaining thorough documentation.

• Conduct follow-up sessions, home visits, and observations, ensuring continuous support for patient needs and adjustments to treatment plans as necessary.

• Serve as a liaison with the mental health community, aiding in development goals and collaboration for client welfare.

• Assist in formulating training materials to enhance program effectiveness for individuals in treatment.
