Responsibilities
- Deliver physical therapy services to patients in a home-based, transitional care context
- Conduct initial evaluations, develop and implement individualized plans of care, and adjust treatment as needed
- Administer evidence-based interventions (therapeutic exercise, gait and balance training, ROM, strengthening) and monitor progress
- Document patient assessments, treatments, and outcomes in EPIC and Cerner with accuracy and timeliness
- Collaborate with care teams, physicians, and family/caregivers to optimize safety and functional independence
- Provide patient and caregiver education on home exercise programs, mobility, fall prevention, and safety strategies
- Ensure compliance with regulatory and payer guidelines and maintain high standards of patient safety and confidentiality

Requirements
- Recent experience using EPIC and Cerner systems
- Experience delivering PT services in a home health or equivalent setting
- Availability for a day shift, 40 hours per week, on an 8-week assignment
- Ability to work in a home-based clinical environment and participate in a system-wide EPIC transition
- Strong communication and collaboration skills to coordinate with the care team and patients
