Case Manager - Supportive Housing

Position Summary

The Case Manager - Supportive Housing is responsible for providing high-quality case management services to clients in compliance with funding guidelines and agency policies and procedures; for providing services in the Engagement Center; completing effective record keeping, and collaborating with the multidisciplinary teams in supportive housing and center programming.

The Case Manager - Supportive Housing reports to the Case Manager Lead - Supportive Housing. 

Position Responsibilities

Program Delivery (70%)

• Provides comprehensive case management services to assigned clients and their families to support stabilization in housing, maintenance or increase of income, and increase in skills and resources
• Complete intake, assessments, service plans, annual progress assessment, benefits assessment.
• Provide hands-on life skills instruction in formal and informal settings, possibly including, but not limited to financial literacy, tenant education, parenting education, and health/wellness education and access, and other skill development as needed
• Collaboratively develops and updates individual service plans with clients, provides encouragement, education, and advocacy toward clients’ individual goals
• Provides or supports mediation for issues that arise between client and landlord, family, or other third parties
• Utilizes motivational interviewing and harm reduction strategies to support movement toward housing stability, increased skills and income, improved health, and achievement of personal goals
• Provides case management services in the Engagement Center including but not limited to welcoming clients, running groups, coordinating food and coffee access, helping clients with engaging in and accessing center resources, and facilitating relationships between clients.
• Provide crisis intervention as needed
• Connects clients to community resources - attending appointments with clients when appropriate - and completes referrals for services identified in case notes or service plans
• Coordinate and/or provide transportation as appropriate for client, including transporting clients in personal vehicle as appropriate
• Assists in the bringing potential linkage agreements with service providers to supervisor
• Distributes transportation assistance, food assistance, and donated goods in accordance with agency policy
• Conducts follow up with past clients in accordance with funder guidelines

 

Program Administration (30%)

• Documents all client contacts and all services provided in progress notes
• Independently maintain organized physical, digital and HMIS client files according to Supportive Services’ record keeping policies
• Maintains updated paper files in neat, chronological and legible condition
• Completes all required paperwork in an accurate and timely manner, including but not limited to: assessments, service plans, contact log, compliance form, and Christmas basket forms
• Completes timely and accurate data entry into HMIS and any additional database as needed according to agency policy
• Completes annual and closing documentation including final service plans, exit form and HMIS interim assessments

 

Qualifications  

• Bachelor’s degree in social work/psychology/related human service field (preferred) or two years of related experience
• One year of case management experience (preferred)
• Excellent interpersonal and interdisciplinary collaboration skills
• Ability to work flexible hours. Evening and weekend work may be required
• Bilingual in Spanish and English a plus
• Proficient in use and knowledge of Google Suite of products and general office equipment
• Strong interpersonal and communication skills with the ability to work effectively with persons who may be ill, disabled, emotionally dysregulated and/or agitated through crisis intervention and de-escalation techniques
• Ability to show empathy, culturally responsive care while remaining nonjudgmental
• Ability to make independent judgments and escalate needs to appropriate support staff
• Ability to manage multiple tasks
• Training and/or experience in use of Motivational Interviewing, Trauma-Informed and Harm Reduction techniques
• Commitment to the mission of ReVive Center

Working Conditions and Physical Requirements:

• Travel between client residence and location(s) in the community to provide contextually relevant services
• Work is normally performed in engagement center, office or client residences
• Conducts independent home visits and appointments in the community with clients
• Sitting for extended periods is required
• Intermittent physical activity including standing, walking, bending, climbing stairs, lifting (less than 10 pounds)
• Functional hearing, vision, manual dexterity and hand-eye coordination or effective accommodations in place to perform job functions
• Effective verbal communication
