Heart to Heart Hospice

Heart to Heart Hospice in Indiana is seeking a Patient Care Manager - Registered Nurse to lead clinical departments and ensure continuity, quality and safety of services in line with state and federal regulations. This role requires coordinating care and guiding a multidisciplinary team to deliver compassionate hospice care in the community.

You will supervise nursing staff, oversee Plan of Care assessments and re-assessments, and provide coaching and evaluations, while maintaining timely
