Nurse Practitioner/Physician Assistant

We are seeking a Nurse Practitioner or Physician Assistant to join a dedicated adult primary care practice located within a comprehensive healthcare facility. In this role, you will provide primary care services for patients aged 18 and older conduct wellness visits, preventive screenings, and routine exams evaluate, diagnose, and treat acute and chronic conditions such as hypertension, diabetes, COPD, and obesity manage lab, diagnostic, and imaging results develop personalized treatment plans emphasizing prevention and disease management educate patients on medications and lifestyle changes collaborate with physicians and care teams to coordinate care utilize telemedicine and patient portals to enhance engagement maintain accurate documentation within electronic health records Qualifications include current NY licensure as an NP or PA, graduation from an accredited program, DEA eligibility, and strong clinical skills. Preferred experience involves internal medicine, primary care, or managing chronic diseases; new graduates with a strong interest are encouraged to apply.
