Clinical Case Coordinator Full time Salaried PROFESSIONAL Bronx E147th, The Bronx, NY, US 18 days ago Requisition ID: 2371 Salary: $69,010.00 Annually Job Title: Clinical Case Coordinator Job Summary: CASES is currently seeking a highly organized and detail‑oriented Clinical Case Coordinator for our Intensive Services Unit. Rapidly after release into the program, the Clinical Case Coordinator conducts Supervision Introduction/Orientation meetings with new participants to review supervision conditions that will be required under the supervision plan. The Coordinator assists participants on their caseload to comply with supervision appointments and court appearances, conducts ongoing service‑need assessments, develops collaborative service plans, and ensures participants receive services that are strength‑based, gender‑specific, and culturally sensitive. The role requires working closely with a Peer Specialist and a coordinated clinical team, supporting participants in achieving their treatment, recovery, rehabilitation, and crime‑free community integration goals. Responsibilities Provide supervision and community‑based services to a caseload of pretrial participants. Within 24–48 hours of release conduct Supervision Introduction/Orientation meetings to review supervision conditions, the reporting schedule, and communication methods required under the supervision plan. Collaborate with the court and pretrial staff to ensure participants successfully complete mandated supervision appointments and court appearances. Review legal and court screening documentation, analyze offending behavior patterns, and identify supports that may reduce the likelihood of pretrial arrests. Conduct ongoing service‑need assessments to examine psychosocial needs, severity of mental and substance use disorders, and motivation for treatment. Link participants to appropriate treatment based on eligibility, preferences and needs, and provide escort to community services. Maintain collaborative relationships with community providers (supported housing, health homes, care management programs, addiction treatment, mental health providers, peer‑run providers, education, vocational training, employment services, legal services, medical care, and family providers). Report any hazards or unsafe conditions to appropriate facilities staff or supervisor. Perform any other duties required by the Program Director or supervisory staff. Qualifications Master’s degree in social work, mental health counseling or comparable professional degree. Minimum of three years’ experience in human services with individuals with substance use disorders, co‑occurring mental health and substance use disorders, and involvement in the criminal justice system. Extensive knowledge of community treatment, support services and resources. Knowledge of the criminal justice system, court processes and alternative to detention/incarceration services. Experience using databases such as Salesforce. Must be able to sit for extended periods. Preferred Skills (Not required) NYS licensure preferred. Bilingual Spanish and English preferred.
