Clinical Case Coordinator I (MSW) Full time Salaried • 4 West 125th Street, New York, NY, US Requisition ID: 2364 • Salary: $69,010.00 Annually Job Title: Clinical Case Coordinator Job Summary: CASES is currently seeking a highly organized and detail-oriented Clinical Case Coordinator for our Intensive Services Unit. The Clinical Case Coordinator conducts Supervision Introduction/Orientation meetings with new participants to review supervision conditions that will be required under the supervision plan and supports participants through their case load to comply with supervision appointments and court appearances during the pendency of their legal case. The role involves ongoing service need assessments and the development of collaborative service plans addressing identified needs and goals. Services are delivered in a strength-based, gender-specific, and culturally sensitive manner while adhering to the core principles of risk, need and responsivity to reduce recidivism, increase engagement in community treatment services, and support participants to achieve their self-identified goals for treatment, recovery, rehabilitation, and crime-free community integration. The position works as part of a coordinated clinical team and collaborates with a Peer Specialist. It is based in the program’s borough office with field work in the greater NYC area as needed. The ideal candidate will possess excellent organizational and communication skills and the ability to prioritize tasks efficiently. This initiative will implement innovative approaches to working with individuals in the Supervised Release Program who have more extensive needs and challenges that may impact daily functioning and program engagement. The focus group of program participants manage challenges in mental health, substance use, employment, and medical health. The team promotes proactive, sustained engagement in the community by bridging participants to community partners and resources to support their potential. Salary: The salary for the Clinical Case Coordinator role is $69,010 per year. Shift Hours: Monday through Friday, 9:00 am to 5:00 pm. Location Address: 4 West 125th Street, New York, New York 10027. Workplace Flexibility: Hybrid: Staff may work remotely one day per week after successfully demonstrating an understanding of their job responsibilities following the first 30 days of onboarding. What you will be doing: Provide supervision and community-based services to a caseload of pretrial participants. Rapidly (within 24-48 hours of release) conduct Supervision Introduction/Orientation meetings with new participants to review supervision conditions, the reporting schedule (as assigned by level of recidivism risk), and the mandatory methods of communication (face-to-face, telephone, and texts) that will be required under the participant’s supervision plan. Work collaboratively with the court and other pretrial staff to facilitate and ensure caseload participants’ successful completion of mandated supervision appointments and court appearances during the pendency of their legal case. Review legal and court screening documentation and work with participants to analyze offending behavior patterns, identify behavioral changes, and determine supports needed to reduce the likelihood of pretrial arrests. Conduct ongoing service need assessments with participants to examine psychosocial needs and problems, including the severity of mental and substance use disorders, participation barriers, engagement, and motivation for treatment. Link participants to treatment based on eligibility, preferences, and needs and provide escort to community services. Maintain collaborative relationships with community providers (housing, Health Homes, care management, addiction and mental health services, peer-run providers, education, vocational training, employment services, legal services, medical care, and family services). Ensure a safe working environment by reporting hazards or unsafe conditions to the appropriate facilities staff or supervisor. Perform other duties as required by the Program Director and supervisory staff. What we are looking for: Master’s degree in social work, Mental Health Counseling or comparable professional degree. Minimum of three years’ experience in human services with individuals with substance use disorders, co-occurring mental health and substance use disorders, and involvement in the criminal justice system. Extensive knowledge of community treatment, support services and resources. Knowledge of the criminal justice system, court processes, and alternatives to detention/incarceration. Experience using databases such as Salesforce. Must be able to sit for extended periods.
