Physical Therapist

Catholic Health Services is seeking a compassionate Physical Therapist for our Catholic Home Health team. In this field-based role, you will provide in-home evaluations, develop individualized treatment plans, and deliver evidence-based interventions to improve mobility, function, and quality of life. You will collaborate closely with an interdisciplinary team, document care in EHR, and educate patients and caregivers. Responsibilities Evaluate home health patients' functional status and mobility needs Develop and implement individualized evidence-based care plans Provide therapeutic exercise, manual therapy, and mobility training Educate patients and caregivers on home exercise and safety Document assessments, goals, and outcomes in EHR accurately and timely Collaborate with nurses, physicians, and interdisciplinary team members Ensure compliance with agency, state, and federal regulations Incorporate faith-based, holistic principles into patient care when appropriate Participate in case conferences and quality improvement initiatives Provide safe care in patients' homes, adhering to infection control standards Required Skills Patient assessment Treatment planning Therapeutic exercise Manual therapy Home health care planning Gait and balance training Electronic health records (EHR) documentation Interdisciplinary care coordination Patient and family education Infection control compliance #J-18808-Ljbffr Catholic Health Services
