Home Care Field Registered Nurse (RN) – Fulton County, NY | $85K

At Home Care Partners is seeking an experienced and compassionate Registered Nurse (RN) to join our field-based clinical team serving clients throughout Fulton County, New York .

This full-time position offers an opportunity to step outside the traditional facility environment and provide individualized nursing support directly in patients' homes and community settings. The Field RN works closely with clients, families, caregivers, and aides to promote safe, consistent, and high-quality care.

Key Responsibilities

• Complete in-home nursing assessments throughout Fulton County

• Conduct head-to-toe assessments and medication and diagnosis reviews

• Perform aide supervisions and provide clinical guidance

• Develop, review, and update Plans of Care (POC)

• Orient aides to client-specific Plans of Care

• Monitor and report changes in client condition

• Maintain accurate and timely clinical documentation

• Collaborate with patients, families, caregivers, aides, and office staff

• Maintain compliance with applicable healthcare regulations
  

Requirements

• Current New York State RN license

• Ability to travel throughout Fulton County

• Medical Professional Liability insurance

• Strong assessment, documentation, and communication skills

• Comfortable using Gmail and Microsoft Word/Excel

• Ability to work independently

• Must meet all company hiring and onboarding requirements
  

Preferred: Home care, community health, or outpatient nursing experience; Medicaid, Medicare, or MLTC knowledge; Plan of Care experience; bilingual Spanish/English.

Salary: $85,000 per year
