Riverbank Post Acute is a 99-bed Medicare and Medi-Cal certified skilled nursing facility and is one of the newest additions to the American Specialty Management group based in Modesto, California.

Overview of the Position: 

The MDS Coordinator assumes full time administrative authority, responsibility and accountability to coordinate the completion of the required minimum data set and with other members of the inter-disciplinary team, develops and implements a plan of care that meets the individual needs of each resident.

• Provide quality, resident-centered care while protecting resident rights and confidentiality.
• Follow facility policies, HIPAA, and all applicable regulations.
• Maintain knowledge of long-term care practices, admissions, reimbursement, and MDS/RAI processes.
• Communicate effectively with residents, families, staff, and regulatory agencies.
• Follow safety, infection control, and emergency procedures.
• Prioritize tasks, maintain professionalism, and complete assigned duties.
• Perform other duties as assigned.

QUALIFICATIONS/REQUIREMENTS:

• Education:  Graduate of an accredited school of nursing 
  
• License:  Current active license as a Registered Nurse or Licensed Vocational Nurse (LVN)
  Work Experience: At least 6 months to 1 year of MDS experience
• SNF experience preferred
