Key Responsibilities

• Provide psychological support and counseling to patients and their families, addressing emotional and social factors contributing to their health status.
• Facilitate patient education initiatives, helping individuals understand treatment options and care paths to encourage active engagement in their health journey.
• Advocate for patient rights and ensure access to necessary protective services and community resources.
• Collaborate with an interdisciplinary healthcare team to develop and implement tailored treatment plans, ensuring coordinated care that aligns with the specific needs of each patient.
• Utilize electronic medical records system to document patient progress effectively and communicate updates to relevant healthcare providers, ensuring a seamless transition of care.
• Perform necessary assessments to identify at-risk patients, utilizing appropriate tools and technology to intervene and mitigate potential readmission risks.
• Participate in transition conferences, providing insights and support to patients and families as they navigate healthcare changes.
• Assure adherence to ethical practices and legal requirements by reporting any suspected cases of vulnerability or abuse.
• Engage in ongoing professional development to enhance therapeutic skills and stay current with best practices in mental health treatment.
• Fulfill additional duties as assigned to support clinic operations and patient care needs.

Patient Population / Client Focus

The position seeks a therapist with experience working with diverse populations, including individuals with acute, long-term, or terminal illnesses. The therapist will address a range of psychological conditions and the unique challenges facing patients and families in these situations. This role’s advocacy and support will empower clients to navigate their healthcare experiences, reducing feelings of isolation and promoting resilience during difficult times.

Qualifications

• A Master's or Doctorate degree in Clinical Psychology, Counseling, or Clinical Social Work (e.g., MA, MS, MSW, MFT, MHC, PhD, PsyD) from an accredited institution.
• An independent license in the state of residence (LCSW, LMFT, LPC), valid in the United States.
• Proficiency in using electronic medical records and communication systems, showcasing efficiency in managing patient information and documentation.
• Strong communication skills, both written and verbal, that support effective collaboration with patients and healthcare team members.
• A demonstrated passion for impacting the lives of patients through evidence-based therapeutic practices and empathetic care.

Preferred Experience

Compensation and Benefits

The compensation for this position ranges from USD 100,000 to 105,000 annually, commensurate with experience and qualifications.
