Responsibilities

• Administer prescribed medications and treatments according to the care plan and facility policies.
• Monitor residents’ health status, document vital signs, and escalate changes to the supervising nurse promptly.
• Provide direct patient care including assistance with activities of daily living, mobility support, and hygiene.
• Assist with wound care, routine procedures, and routine nursing tasks within the LPN scope of practice.
• Maintain accurate and timely documentation in the electronic medical record; ensure accuracy and privacy of resident information.
• Collaborate with the interdisciplinary team to implement and adjust care plans; uphold safety and infection control standards.

Requirements

• Active Licensed Practical Nurse license to practice in New Mexico
• Current CPR/BLS certification (American Heart Association or equivalent)
• Ability to work 12-hour shifts in a 3x12 schedule
• Experience in long-term care or skilled nursing setting preferred
• Education: Practical Nursing diploma or equivalent
• Local assignment eligibility: must be available for a local Gallup, NM assignment
