Key Responsibilities\n

• Conduct individual and group therapy sessions tailored to the emotional needs of patients and their families, using evidence-based counseling techniques.

• Assess and address the mental health needs of patients experiencing grief, anxiety, and other psychological effects associated with hospice care.
\n
• Document patient interactions, progress notes, and treatment plans accurately and in a timely manner, adhering to best practices in clinical documentation.

• Facilitate multidisciplinary case reviews and collaborate with healthcare team members to develop comprehensive care plans tailored to the unique needs of each patient.

• Provide crisis intervention support during critical moments, ensuring patients and their families receive immediate and compassionate care.

• Educate patients and their families about coping strategies for dealing with life-limiting illnesses and grief.
\n
• Participate in regular training and development to stay current with best practices in hospice care and mental health.

Patient Population / Client Focus\n

This role primarily focuses on working with patients in a hospice setting, along with their families, during one of life’s most challenging transitions. You will be supporting individuals coping with terminal illnesses, addressing both their psychological needs and the emotional challenges faced by family members. Your work will directly contribute to enhancing the quality of life for patients, supporting their preferences, and promoting dignity in their final days.

Qualifications\n

• A Master's degree in Social Work from an accredited institution.

• An active and current Licensed Clinical Social Worker (LCSW) license in the state of Pennsylvania.
\n
• A minimum of 2 years of experience in a clinical social work setting, specifically within outpatient mental health or hospice environments.

• Proven proficiency in medical documentation and record-keeping using electronic health records (EHR).

Preferred Experience\n

• Training in motivational interviewing or other therapeutic techniques.

• Experience working with children and/or adolescents is a plus.
\n
• Background in crisis intervention and management in mental health settings.

• Experience addressing gender issues and providing gender-sensitive counseling.

Compensation and Benefits\n
