Job Description
Case Management - Acute Care

Schedule

Weekends: Every other weekend (EOW). Call: Not required. Floating: May float to sister facilities as needed. Pre-approved time off: submit all requests upfront (two allowed). Holiday commitment: Must work 3 of the 5 core holidays.

Assignment Details

Daily caseload: up to 25 patients.

Requirements

• Valid state license required; compact license must be in-hand at submission.
• Minimum 1 year of acute hospital case management experience within the last year.
• Experience with discharge planning, care coordination, benefits eligibility, prior authorizations, and appeals/denials.
• Familiarity with DRG, core measures, and HIPAA.
• Charting system: Epic required.
• References: 2 required (covering 1 year within last 3 years).

Responsibilities

• Conduct discharge planning and care coordination for diverse patient populations.
• Ensure benefits eligibility and handle prior authorizations.
• Manage appeals and denials effectively.
• Collaborate with interdisciplinary teams, including social services.

Compliance / Notes

Must provide all scheduling requests at submittal; post-offer requests may not be granted.
