Job Opportunity

Position Overview

We are seeking a qualified candidate with 2 years of acute hospital case management experience to join our team. This role involves monitoring up to 2 units over weekends with a primary focus on discharge planning.

Key Details

Shift: Day Shift, 5x8-Hour Schedule (08:00 AM - 04:30 PM)
Location: City, State
Start Date: As soon as possible
Weekend Requirement: Every other weekend
Holiday Coverage: Must be available for holiday shifts
Current State License: Preferred; pending licenses accepted

Responsibilities

Coordinate and oversee discharge planning for patients across multiple units.
Provide care coordination and assist patients and families with navigating care processes.
Conduct biopsychosocial assessments and ensure documentation according to care plans.
Educate patients and caregivers to facilitate effective discharge processes.
Collaborate with a multidisciplinary team to ensure continuity of care.

Qualifications

Minimum 2 years of acute hospital case management experience required.
Experience with the Epic charting system preferred.
Knowledge of drug/alcohol treatment options and mental health crisis intervention.
Ability to participate in family meetings and treatment plan development.
Strong communication skills and experience working with diverse patient populations.

Unit Details

Patient Age Groups: Neonates, Adolescents, Infants, Adults, Pediatrics, Geriatrics
Daily Caseload: Up to 25 patients
Additional Support: Access to interdisciplinary teams, social services, pharmacology, and respiratory services.
