What will I do as the Coordinator of Rehabilitation Services for The Wesley Community?

The Coordinator of Rehabilitation Services plans, organizes, and supports daily rehabilitation operations and ensures efficient delivery of therapy services. The position provides direct patient care within the scope of practice as a COTA or PTA and serves as a liaison among therapy staff, nursing, residents, families, and facility leadership. Provide treatment interventions, implement care plans, document resident progress, participate in care conferences, and support resident-centered rehabilitation services.

A few of the most common duties and responsibilities required of The Coordinator  include:

*Assists the DOR as a liaison between staff & facility administration, and is able to communicate assertively, diplomatically and clearly.

* Actively involved in facility and department meetings, as needed, to help promote both facility goals and rehab goals and ensure the needs of all residents are being satisfactorily met while remaining conscious of fiscally responsible time utilization.

*Collaborate with appropriate facility staff on the purchasing process for supplies and equipment.

*Orientation and training of rehabilitation personnel.

*When needed, assist DOR with projecting minutes, scheduling groups and concurrent, reviewing documentation Due list, and setting daily therapist hours. 

*Supervision of clinical rehabilitation and support staff within the facility.

*Conduct facility audits to ensure compliance with equipment, orders, care plans, etc. and develop action plans for frequent noncompliance.

*Reviewing referrals and discharges of Rehabilitation and Long-term care programming to evaluate service delivery and needs.

* Cover for DOR duties during scheduled time off.

*Maintains 75% productivity daily

*Creates a work atmosphere that promotes cooperation, respect, flexibility and teamwork amongst peers and staff.

Requirements :

*Associate degree from an accredited PTA or Occupational Therapy Assistant program.

*Minimum two years of rehabilitation experience. Skilled nursing experience preferred.

*Current unrestricted PTA or COTA license required. CPR certification is preferred.

*Strong knowledge of Medicare Part B services.

*Experience with Point Click Care and Net Health therapy platforms.
