Case Manager – San Jose, CA Salary: $27.75 to $33.83 per hour Full Time, Non-Exempt, Driving Required About PATH Since its foundation in 1984, PATH (People Assisting The Homeless) has pioneered bold and effective approaches to assist people experiencing homelessness. PATH operates services throughout California, connecting clients to a comprehensive continuum of homelessness prevention, street outreach, employment preparation and placement assistance, individualized case management, supportive services, interim housing, and permanent supportive housing. About the Job As part of the Santa Clara team, the Case Manager will assist clients in breaking the cycle of homelessness by utilizing a “whatever it takes” approach. The role supports clients through all phases of homelessness—on the street, interim housing, transitional housing, and permanent housing—helping them access and maintain necessary services in healthcare, social services, interpersonal support systems, and through a coordinated entry system (CES). The position provides individualized supportive services and helps each client develop a plan to address strengths, barriers, health needs, income, and housing. Position Responsibilities Conduct intake and individualized needs assessment for all participants and collaborate to develop an Individualized Service Plan (ISP) that addresses housing goals, increasing income, and other personal goals identified by the participant, including medical, mental health, substance use, financial resources, vocational, and social support needs. Provide referrals, linkages, information, and support to resources that help participants achieve their ISP goals. Complete program‑specific assessments (e.g., VI‑SPDAT). Evaluate strengths and challenges to addressing short‑term and long‑term goals, conduct a 90‑day review with the participant after initial intake. Maintain documentation standards as required by the Program and PATH policies, including completing case file notes in a timely manner and maintaining a complete, accurate, and current participant file with all required documents. Maintain confidentiality of participant files per HIPAA and all applicable guidelines. Develop therapeutic and/or professional relationships with clients and systems/partners to enhance efforts to engage and retain individuals with their housing goals. Provide crisis intervention and risk assessment services focused on increasing safety and enhancing the client’s ability to independently solve problems, utilize effective coping skills, and manage and self‑coordinate their own care, including services for individuals with chronic trauma. Use motivational interviewing and trauma‑informed care in service delivery. Maintain current, thorough knowledge of community resources and utilize them to provide comprehensive, wrap‑around services to clients. Ethics and Boundaries: maintain ethical and professional standards, demonstrating honesty and integrity while maintaining professional boundaries. Preferred Qualifications Two (2) years of experience in healthcare, case management, homeless services, or working with vulnerable populations or a related field. Minimum Qualifications All levels of education and experience are welcome. Mandatory Requirements Have employment eligibility verification. Have or be able to obtain CPR/First Aid training. Successfully complete the following as a condition of hire: Tuberculosis test. Background screening. Drug test. Driving All candidates must have reliable transportation, a valid driver’s license, proof of insurance and ownership for personal vehicles used during work duties, and the ability to qualify for PATH's insurance coverage.
