What You'll Do Evaluate homebound patients referred for physical therapy and identify functional needs. Develop and implement individualized treatment plans based on each patient's goals and condition. Provide therapeutic exercise and other appropriate interventions to improve strength, mobility, balance, endurance, coordination, gait, and daily function. Assess pain, joint mobility, motor control, posture, cardiopulmonary function, soft-tissue integrity, and the ability to complete activities of daily living. Educate patients and caregivers on treatment plans, safe mobility, home exercise, and strategies that support independence. Collaborate with the care team to coordinate services, monitor progress, and adjust the plan of care as needed. Complete timely, accurate documentation in accordance with clinical and organizational standards.

Qualifications PT experience required. Graduate of Physical Therapy Program from an accredited American Physical Therapy Association school, BS degree or entry level master's degree. Active CT PT license. CPR certification required at time of start.

Regular Standard Hours Per Week : 40 Schedule : Full-time (40 hours) Shift Details : Mon thru Friday Float position for greater Torrington area. Requires 1 weekend day per month and 1 major or minor holiday per year coverage. Will consider less than 40hrs per week position
