Role Overview As the Care Transitions Coordinator (CTC), you will be responsible for executing our sales strategy to increase market share through account development and educating the medical community about our services while operating within budget. The CTC primarily facilitates a seamless transition for patients discharged from a facility to our agency for post‑acute care. Primary Responsibilities Achieve monthly Personal Production Goals and MC admit budgets for assigned locations while stewarding financial resources by projecting ROI and managing within the Sales and Marketing expense budget. Execute weekly, monthly, and quarterly strategies to increase market share within assigned facilities. Evaluate patients and orders for suitability for home care following Right of Choice. Initiate face‑to‑face patient transition and educate the patient on our agency; identify the primary care physician to follow the plan of care. Present patient needs to the agency Executive Director for branch approval and complete CTC encounter documentation in Home Care Home Base. On acceptance, coordinate transfer orders, ancillary services (DME | Infusion) as needed, and educate patients on home‑care/hospice orders. Ensure all patient needs from the referral source are documented and met by the agency. Collaborate with the Executive Director/Clinical Director to drive growth by focusing on the referral community and patient expectations. Handle all sales administration duties, including BOA expense entry compliance, payroll time sheets, weekly 3LS meetings, PTO requests, and timely communication. Educate patients on post‑facility discharge follow‑up appointments, medication prescriptions, and pharmacy details. Serve as liaison between the agency and all involved healthcare providers for newly referred and transferred patients. Communicate with discharge planning about active patients transferring from home health into a facility and coordinate resumption of care before discharge. Provide case‑management feedback on readmissions and non‑admit decisions. Maintain patient confidentiality at all times. Know the features and benefits of the services provided by LHC Group, articulate competitive advantages, specialty programs, and Medicare guidelines, and educate the medical community through sales calls and in‑services. Required Qualifications Current and unrestricted RN, LPN, SW, or PT licensure in the state of practice. Valid driver's license, vehicle insurance, and reliable transportation or public transit access. RT and/or technical school certification demonstrating solid clinical knowledge. At least 1 year of home health or hospital case management experience. Thorough understanding of home health qualifying criteria and coverage guidelines. Excellent presentation, negotiation, and relationship‑building skills. Solid computer skills, including Microsoft Outlook and related software. Preferred Qualifications One or more years of medical marketing experience. Excellent verbal and written communication skills with the healthcare team. Excellent organizational skills and ability to juggle competing priorities. Salary & Benefits Salary ranges from $56,012 to $84,018 annually for full‑time employment. Pay is based on several factors including local labor markets, education, work experience, and certifications.
