Job Responsibilities

• Conduct comprehensive evaluations for patients in inpatient or outpatient environments and formulate customized treatment plans
• Implement physical therapy interventions focusing on mobility, strength, balance, and overall function
• Work in partnership with nurses, physicians, case managers, and other rehabilitation professionals to achieve optimal patient outcomes
• Educate patients and their families on therapeutic objectives, discharge conditions, and home exercise regimens
• Maintain detailed documentation of evaluations, treatments, and progress in line with facility policies and regulatory standards
• Engage in care team rounds and interdisciplinary meetings to ensure cohesive patient care

Qualifications

• Doctorate in Physical Therapy (DPT) or equivalent degree from an accredited institution

Certifications

• State licensure required for Physical Therapist
• CPR certification is advantageous
• Specialized certifications (e.g., neurological, geriatric) are a plus but not mandatory

Qualifications and Skills
