Key Responsibilities

• Oversee clinical workflows to ensure efficiency in patient referrals, processing, and scheduling, optimizing patient capture rates.
• Develop and execute individualized treatment plans for each patient, focusing intently on early intervention and conducting same-day evaluations when necessary.
• Promote a culture of collaboration by fostering an inclusive environment that encourages contributions from all team members, resulting in improved quality of care and patient safety.
• Collaborate closely with our Director of Therapy Operations to identify opportunities for clinical improvements and implement innovative practices.
• Build and maintain strong relationships with employers, referral sources, and payer groups through regular communication and engagement activities, ensuring seamless support for our patients.

Patient Population / Client Focus

Our typical patients include individuals recovering from surgeries, elderly patients needing rehabilitation after falls, as well as those with chronic illnesses and disabilities. By delivering care directly in the patient's home, you will have the unique opportunity to address their specific living situations and tailor your approach to their daily challenges. This proximity allows for real-time assessment and adjustment of therapy plans, ensuring optimal recovery outcomes.

Qualifications

• Current and active Physical Therapist license in the state of Florida.
• A Bachelor's, Master's, or Doctorate degree in Physical Therapy from an accredited institution.
• Exceptional communication and interpersonal skills, with a talent for connecting with patients of diverse backgrounds.
• Strong organizational skills and the ability to manage your time effectively in a fast-paced home care environment.
• Critical-thinking and problem-solving abilities that enable you to address challenges and adapt therapy as needed.

Preferred Experience

• Previous experience working in home care or outpatient therapy settings is highly desirable.
• Background in a specialty area, such as geriatrics, orthopedics, or neurological rehabilitation.

Compensation and Benefits

• Competitive annual salary between $120,000 and $130,000 , commensurate with caseload and experience.
