Work Environment

The Physical Therapist will work onsite in various home settings across Palm Bay. Home care modalities provide a unique environment that allows for one-on-one patient interactions, creative treatment adjustments in familiar settings, and direct engagement with patients' families. Key Responsibilities

• Conduct thorough evaluations of patients' medical histories and perform detailed physical examinations to inform tailored, personalized treatment plans.
• Implement treatment strategies that may include exercises, manual therapy, and patient education to facilitate rehabilitation and recovery.
• Continuously monitor patients' progress, making real-time adjustments to treatment protocols based on their evolving needs and feedback.
• Ensure accurate documentation of treatment plans, patient outcomes, and progress notes to maintain compliance with healthcare regulations and standards.
• Educate patients and their families on effective self-management techniques, safety measures, and preventive strategies to avoid re-injury or worsening of conditions.
• Collaborate with multidisciplinary healthcare teams to provide comprehensive, integrated care, sharing insights and recommendations for comprehensive health improvement.

Patient Population / Client Focus

Our patients range from post-operative recovery individuals to chronic condition management seekers, including elderly patients experiencing mobility challenges and younger individuals recovering from injuries. The nature of home care allows for personalized attention, focusing on the unique needs and goals of each patient in the comfort of their own home.

Qualifications

• Valid and active Physical Therapist license issued by the Florida State Board of Physical Therapy.
• Completion of an accredited Bachelor's, Master's, or Doctoral degree in Physical Therapy.
• Strong interpersonal communication skills to effectively connect with patients and their families.
• Proficient organizational skills to manage treatment documentation and schedules efficiently.
• Demonstrated ability to adapt in a fast-paced, changing environment while employing critical thinking and problem-solving skills.

Preferred Experience

• Previous experience in home health or outpatient physical therapy settings is highly desirable.
