RN Care Navigator: Discharge & Care Coordination

You will collaborate with physicians, payers, and the care team to coordinate care, ensure appropriate level of care, and optimize patient outcomes. The role requires an RN license in Michigan, 2 years in acute care, and 3–5 years in care management or discharge planning. BLS and CCM credentials are preferred.
