Work Environment

As a Physical Therapist at Heartfelt Insights, you will primarily work onsite in the homes of patients. This home care setting allows for personalized therapy sessions where you can build strong relationships with your patients and witness their progress up close. The support from our interdisciplinary team, including physicians, nurses, and occupational therapists, plays a crucial role in delivering comprehensive care tailored to each patient's needs.

Key Responsibilities

• Conduct thorough evaluations of patients to identify impairments, functional limitations, and disabilities.
• Design and implement individualized treatment plans based on the physician's orders, ensuring that patients receive targeted and effective interventions.
• Document patient assessments and treatment outcomes meticulously, ensuring compliance with healthcare standards and regulations.
• Maintain and manage patient records, billing charges, and therapy notes in a timely manner.
• Supervise and provide guidance to Physical Therapy Assistants (PTAs) and support personnel, ensuring collaborative patient care.
• Proactively engage with other healthcare professionals to discuss patient progress and optimize treatment plans.
• Provide feedback to management and contribute to quality improvement initiatives within the organization.

Patient Population / Client Focus

Your role will involve working with a diverse patient population, including individuals recovering from surgery, injuries, or chronic conditions that impact physical abilities. You will cater to a wide age range and ensure that therapeutic goals are aligned with the patients' personal health aspirations, thus enhancing their overall quality of life.

Qualifications

• Bachelor's Degree in Physical Therapy or a related field.
• Current licensure as a Physical Therapist in the state of Virginia.
• Strong written and verbal communication skills for effective patient and team interactions.
• Proficiency in documenting patient information and treatment plans using electronic medical record systems.

Preferred Experience

• A minimum of two years of experience in a physical therapy setting, preferably focusing on home healthcare.
• Experience with diverse patient populations, particularly in geriatric and post-operative care.

Compensation and Benefits

The salary for this full-time Physical Therapist position ranges from $80,000 to $100,000 annually, based on experience and qualifications.
