Key Responsibilities

• Provide high-quality, patient-centered physical therapy by overseeing the full continuum of care for an assigned group of patients.
• Conduct thorough patient assessments and collaborate with the interdisciplinary care team to develop and implement individualized care plans that promote goal achievement and enhance patient engagement.
• Administer therapeutic interventions in accordance with the physician's treatment protocols, including but not limited to, manual therapy, exercise prescription, and patient education.
• Utilize evidence-based practices and treatments to facilitate patient recovery in their home environment, ensuring alignment with established professional and ethical standards.
• Instruct patients, families, and caregivers on effective use of therapeutic exercises, assistive devices, and rehabilitation techniques to maximize recovery and function beyond the clinical settings.
• Manage patient caseload strategically by prioritizing daily responsibilities and coordinating with clinical staff to ensure seamless transition of care.

Patient Population / Client Focus

In this role, you will encounter a diverse range of patient conditions, including post-surgical recovery, orthopedic injuries, neurological disorders, and developmental challenges in children. Emphasis is placed on establishing a trusting relationship with patients and their families, which is essential for fostering adherence to therapeutic regimens and overall health improvement.

Qualifications

• Must possess a Doctor of Physical Therapy (DPT) degree and hold a valid state license to practice physical therapy.
• Basic Cardiac Life Support (BCLS) certification is required to be obtained within 30 days after your start date.
• Strong interpersonal and communication skills to effectively engage with patients, families, and healthcare teams.
• Commitment to ongoing professional development to stay updated with the latest advancements in physical therapy.

Preferred Experience

• A minimum of two years of clinical experience in a relevant setting is preferred, although new graduates are encouraged to apply.
