The Opportunity: Pine Park Health is seeking an experienced, independently licensed Nurse Practitioner (California NP-103 license required) to join our growing team in the East Bay San Francisco market (Oakland, Walnut Creek, Concord). This role is designed for a seasoned NP who is ready to practice at the top of their license - autonomously managing a panel of complex older adult patients within a value-based primary care model, without the constraints of a traditional clinic setting. What you'll do: Provide onsiteprimarycareservices to older adults living in Senior Living Communities Develop and execute detailedperson-centered careplans on allprimarycarepatients Work with Medical Director to identify and develop policies to address issues thatarise which affect day to daypractice, culture or clinical guidelines, including for emerging disease(s) Provide monthly Chronic Care Management (CCM) visits and care; review and sign off on CCM documentation Run regular huddles and provide leadership to the clinical team Identify ways of working withsenior living management andcareteams to better carry out Pine Parkcareplans and improve patient outcomes Your clinical team includes dedicated RNs and Care Coordinators who handle the operational side - so you can stay focused on what you do best, caring for patients. Qualifications: Active California NP 103 Independent Practice License - required AGNP strongly preferred. Final compensation will reflect experience, depth of clinical background, licensure, and geographic market factors. These factors may include but are not limited to education, training, skill set, years and depth of experience, certifications and licensure, business needs, internal peer equity, and alignment with geographic and market data.
