Key Responsibilities\n

• Conduct comprehensive psychosocial assessments by engaging with clients through interviews and reviewing their medical history. This includes evaluating emotional, social, and environmental factors that influence their rehabilitation journey.

• Provide empathetic counseling and emotional support to individuals and their families to address adjustment challenges, coping strategies, and overall mental well-being.

• Collaborate effectively with rehabilitation professionals to create and implement individualized treatment plans that align with the psychosocial needs of patients.

• Assist clients in navigating community resources, which may include support groups, vocational training, and housing assistance to enhance their reintegration into society.

• Provide education and support to patients and their families about available community resources and post-rehabilitation services, helping them understand their options for future care.

• Participate in discharge planning to smoothly transition patients from our rehabilitation services back into their homes or other care settings, ensuring a continuum of care.

Our client population is diverse, encompassing individuals recovering from a range of conditions such as strokes, spinal cord injuries, orthopedic surgeries, and neurological disorders. You will work closely with these patients, providing essential emotional and social support as they navigate their rehabilitation and strive towards achieving their highest potential for independence.

Qualifications\n

• A Master's degree in Social Work from an accredited program is required.

Compensation and Benefits\n
