Job Description Summary

Under general supervision, perform professional case management services for adult offenders (clients) within a residential Work Release facility. Assess clients' individual needs and develop, implement, and monitor transitional and reentry plans to support successful community reintegration. Maintain, analyze, and document data related to service delivery and client outcomes. Research and coordinate community resources and support services, establish linkages with external agencies, and provide case management services in both individual and group settings. Collaborate with security and administrative personnel to support facility operations and ensure compliance with established policies, procedures, and applicable standards.

Monday, Wednesday, Thursday and Friday – 8:00 AM to 5:00 PM, Tuesday – Noon to 9:00 PM.

Job Description

Case Management - 70%

• Orientate clients; review rules and regulations, court orders, assess risks; identify obstacles and strengths using formal and informal assessment tools.
• Assist clients in developing reentry plans into the community based on risk and needs.
• Deliver individual and group instruction in areas such as, cognitive skills building, housing, employment, relapse prevention, life skills, and abilities for prosocial change.
• Maintain files and documentation of program services.
• Work with security personnel and oversee the management and dispensation of medication to clients.
• Establish and maintain a variety of tangible files, filing, and retrieve written or electronic information as requested or otherwise necessary. Utilize computerized data entry equipment and various Word processing, Excel spreadsheet and/or file maintenance programs to enter, store and/or retrieve information as requested or otherwise necessary. Utilize county, state judicial, and NCIC/CCIC databases (through the Justice Services Department) to query criminal case and status information about clientele.
• Input and maintain data to analyze outcomes of client compliance and statistical measurement; evaluate results to improve and/or expand services.

Community Liaison - 15%

• Work with and have frequent contact with judicial, county, community agencies and departmental personnel to document, retrieve or distribute information regarding client status and performance.
• Represent the program at various justice system committees and meetings, and provide program progress reports, as needed.
• Research and gather community resources and provide linkage to outside agencies for client supports.
• Ability to operate safely a vehicle in routine conditions.

Meetings, Trainings & Administration - 15%

• Attend seminars, conferences, workshops, classes, and lectures, as appropriate, to enhance and maintain knowledge of trends and developments in alternative sentencing and departmental services. Keep informed of new statutes, laws and ordinances, case management and counseling techniques, answering questions or inquiries, as needed.
• Attend departmental staff meetings.
• Assist security and administrative personnel with various tasks, ensuring adherence to all policies, processes and procedures.

Required Qualifications

Required Education

• Bachelor's Degree in criminal justice, social work, social sciences, sociology, psychology, counseling or similarly related field.

Experience Qualifications

• 2 years of professional experience in a correctional counseling/case management services position or human services related environment.

Skills and Abilities

• Knowledge of organization, programs, activities and operations of the courts, the department and Weld County; (Medium proficiency)
• Knowledge of individual and group counseling techniques, group facilitation skills, assessment and treatment, understanding of substance abuse, and mental health issues; (Medium proficiency)
• Ability to exercise tact, patience, and courtesy in frequent contact with criminal offenders as well as with various county, court and other public officials with differing levels of authority and influence and the general public; (High proficiency)
• Effective problem solving and decision-making skills; interview skills and understanding of criminal behaviors; (Medium proficiency)
• Strong self-motivation, ability to multi-task and demonstrate flexibility; (Medium proficiency)
• Bilingual (English/Spanish) language skills, with ability to translate, interpret and communicate with others are desired; (Medium proficiency)

• Knowledge of modern office practices, methods and procedures; (Medium proficiency)
• Establish and maintain effective working relationships with judicial and law enforcement officials, Weld County employees, representatives of other agencies and organizations, and members of the community; (Medium proficiency)
• Comprehend frequently used terms in criminal justice and counseling/case management systems; and (Medium proficiency)
• Process and enter volumes of data, with high attention to detail, into various databases, interpret data and effectively use common software applications; query local and state databases. (Medium proficiency)
• Candidate must pass criminal background check prior to employment start date.

Licenses and Certifications

• Driving is essential in this position.
• Candidate must have a valid Driver’s License and Liability Insurance ( Upon Hire Required )
• Candidate must pass a Motor Vehicle Record (MVR) evaluation and if hired, will be subject to continuous monthly MVR monitoring and random drug screens throughout employment. ( Required )
• Pre-employment substance screening is required for this position.
• This position is subject to a pre-employment drug screening. Employment is contingent upon successfully passing the screening in accordance with Weld County policy.

This position is non-exempt from the minimum wage and overtime requirements of the Fair Labor Standards Act and is therefore eligible for overtime pay.
