Responsibilities The Physical Therapist will primarily provide outpatient rehabilitation services in an in‑home setting for the older adult population. The in‑home setting could be a patient’s private residence or a patient’s room in an independent living facility. The Physical Therapist assesses the need for, develops, and delivers physical therapy programs to facilitate rehabilitation. The Physical Therapist identifies the need for and makes referral(s) to other disciplines to address the comprehensive needs of patients. They implement individualized therapy programs designed to restore, reinforce, and enhance performance. The Physical Therapist instructs, educates and trains patients and caregivers in those skills and functions essential for promoting independence and productivity, in order to diminish or correct pathology. They direct activities to promote and maintain health. This position provides a flexible schedule and work environment in various settings. Supervise Physical Therapy Assistants, temporary licensees, students, and support personnel in accordance with state licensure requirements and professional standards. Ensure adherence to department and patient treatment schedule, timely clinical and billing documentation, and participation in work‑related meetings. Attend and contribute to patient care and staffing conferences and other related meetings. Make referrals to other disciplines and agencies to address the comprehensive needs of the patient. Consult with other members of the rehabilitation team to select the most appropriate therapeutic regimen consistent with the needs and capabilities of each patient. Adapt program and/or plan of care according to the needs of the individual patient. Promote a safe environment and enforce the elimination of fire and safety hazards. Order supplies and equipment as necessary. Clean, maintain, identify, and report the need for repair of equipment as necessary. Put patient service first: ensure that patients and families receive the highest quality of service in a caring, compassionate atmosphere that recognizes the individuals’ needs and rights. Screen, examine, and evaluate patients, including history, systems review, and appropriate tests to establish diagnosis, identify impairments, and design a plan of care. Therapeutic exercise including aerobic conditioning, balance coordination, agility, body mechanics, postural stabilization, flexibility, gait, neuromotor training, relaxation, strength, power, and endurance. Functional training Manual therapy Airway clearance techniques Integumentary repair and protection Use of electrotherapeutic, physical agent, and mechanical modalities Health and wellness Education on chronic disease management Falls assessment and intervention to reduce fall risk Case management in home and community Design, fabricate, fit, and/or apply adaptive devices and train patients in their use. Select appropriate activities suited to the individual’s physical and mental capacity, intelligence level, and interest. Instruct patients in skills and techniques required for mobility activities and evaluate progress, attitudes, and behavior. Perform routine re‑examination as needed to modify and progress the plan of treatment. Incorporate health and wellness programming into treatment plans. Perform home and community mobility assessments and provide recommendations for safe mobility. Perform other related duties as required. Qualifications Must have a valid Physical Therapy license in the state(s) of practice, or proof of license eligibility. Must have a Master’s degree in Physical Therapy. Must have a Master’s degree in a Physical Therapy related field in addition to a Bachelor’s degree in Physical Therapy. Must have a Bachelor’s degree in Physical Therapy plus five years of progressively responsible PT experience. The qualifying Physical Therapy degree earned outside of the U.S. must be evaluated to be the U.S. equivalent of a Physical Therapy degree. Must have a valid driver’s license or reliable, independent transportation to and from the patient’s residence. Posted Salary Range: USD $63.00 – $63.00 /hr. Bonus: USD $5,000.00 #J-18808-Ljbffr
