Responsibilities

• Provide direct patient care, including administering medications, wound care, and vital sign monitoring
• Collaborate with interdisciplinary teams to develop and implement individualized care plans
• Demonstrate exceptional communication and interpersonal skills with residents, families, and healthcare professionals
• Participate in quality improvement initiatives and contribute to a positive, patient-centered culture
• Maintain accurate and timely patient records and reports
