Position Summary Syncare Memory Suites is seeking a compassionate, experienced, and highly organized Licensed Practical Nurse (LPN) Clinical Manager to lead daily clinical operations and help deliver exceptional memory care services. This full-time, on-site leadership role is based in Plymouth, Minnesota, and is ideal for an LPN who is passionate about dementia care, resident advocacy, and team development. The Clinical Manager serves as a key member of our leadership team, overseeing resident health services, supervising caregivers, ensuring regulatory compliance, and fostering a culture of excellence in person-centered care. KEY RESPONSIBILITIES Clinical Leadership & Resident Care Oversee and coordinate daily clinical operations within the memory care residence. Conduct resident assessments and monitor changes in health status. Administer medications and treatments in accordance with physician orders and nursing standards. Develop, implement, and update individualized care plans. Collaborate with physicians, pharmacists, therapists, and other healthcare professionals to ensure optimal resident outcomes. Respond appropriately to urgent clinical situations and resident care needs. Supervise, schedule, and support caregiving staff. Provide coaching, mentorship, and performance feedback. Promote a positive, collaborative, and resident-focused culture. Support onboarding and ongoing staff education, particularly in memory care best practices and dementia-specific approaches. Compliance & Quality Assurance Ensure compliance with Minnesota assisted living regulations and company policies. Maintain accurate and timely clinical documentation. Monitor infection prevention and control practices. Participate in quality improvement initiatives and clinical audits. Support survey readiness and continuous regulatory compliance. Family & Community Engagement Build trusting relationships with residents and family members. Provide education, updates, and emotional support to families navigating dementia care. Serve as a clinical resource and advocate for residents' well-being. Qualifications Required Active Licensed Practical Nurse (LPN) license in the State of Minnesota. Minimum of 2 years of nursing experience in assisted living, long-term care, memory care, or a related senior care setting. Strong clinical assessment, medication administration, and care coordination skills. Experience supporting individuals living with Alzheimer’s disease or other forms of dementia. Knowledge of Minnesota assisted living regulations and healthcare documentation requirements. Current CPR certification. Excellent communication, interpersonal, and organizational skills. Ability to effectively lead and motivate care teams while maintaining a compassionate, resident-centered approach. Preferred Previous supervisory or clinical management experience. Experience with electronic health records (EHR) systems. Familiarity with quality improvement initiatives and regulatory surveys.
