Licensed Vocational Field Nurse (LVN)Desire Home Care, Inc. is seeking a compassionate and experienced Licensed Vocational Field Nurse (LVN) to join our team as a per diem healthcare employee. This individual contributor role will be based in the East Riverside area and will involve providing health care services in the comfort of patients' homes. As an LVN Home Health Nurse, you will be responsible for providing individualized and comprehensive care to patients according to their specific health needs, while also adhering to all regulatory and ethical standards.Compensation- Per Diem:Routine Follow-Up Visit: $35-45Wound VAC: $ 40-50Benefits:Daily pay: Access your earned wages the same day you work!Medical, dental, vision401K with 4% matchPaid time offPaid sick timeReferral payPaid trainingsFirst time home buyer program assistanceSign on bonus: Complete 100 visits within 90 days & receive a $500 bonusResponsibilities:Provide skilled nursing care to patients in their homes, in accordance with established care plans and physician's ordersMonitor and assess patients' health status and report any changes or concerns to the supervising Registered NurseAdminister medications and treatments as prescribed by the physicianProvide wound care, catheter care, and other nursing procedures as neededEducate patients and their families on their conditions, medications, and self-care techniquesCollaborate with other healthcare professionals to coordinate and manage patient careMaintain accurate and up-to-date patient records, including progress notes, medication administration records, and nursing assessmentsAdhere to all regulatory and ethical standards, including HIPAA guidelines and infection control protocolsCommunicate effectively with patients, families, and other team members to ensure the best possible outcomes for patientsRequirements: *New Graduates Welcomed*Active and unrestricted LVN license in the state of CaliforniaMinimum of 2 years of experience as an LVN in a home health settingStrong knowledge of nursing principles, practices, and proceduresExcellent clinical assessment and critical thinking skillsAbility to work independently and manage time effectivelyStrong communication, interpersonal, and organizational skillsMust have a valid driver's license and reliable transportationMust have current BLS certificatePhysical and TB clearance valid within the yearAbility to complete documentation via agency EMR systemWork Schedule:Require a minimum of 5-10 visits per weekSelf-determinedEEOC Statement: Desire Home Care, Inc. is an equal opportunity employer and is committed to creating a diverse and inclusive workplace.
