Urology Advanced Practice Provider

Job Summary SUNY Upstate Research Foundation is seeking a highly motivated Advanced Practice Provider who will serve in the Outpatient Clinic working with the Urology Team, providing care to patients at the 550 Harrison Urology Office. Responsibilities Providing expert level clinical care & coordination of care via a full range of services that will improve quality of patient care assessing. Evaluating patients, ordering appropriate laboratory testing & diagnostic studies. Provide outpatient, inpatient, and urological procedures, arranging for referrals, testing, consultation & therapeutic services. Maintaining outstanding relationships with families, referring physicians & all members of the health care team. As an APP, you will be an integral member of the multidisciplinary healthcare team, demonstrating professionalism, leadership, and a commitment to Upstate’s mission, vision, and values, while adhering to all institutional policies and procedures. Minimum Qualifications Master’s Degree as a Nurse Practitioner or Physician Assistant. NYS Licensed/Registered NP or PA. Current National Certification consistent to the clinical practice. NYS licensed/registered NP or PA required. Have a NPI number and be Medicare enrolled with a Medicare Identification Number (or PTAN) (or eligible for Medicare Enrollment within 6 months of hire). CPR-BLS Certification required at the time of appointment. Must successfully pass pre‑employment and credentialing clearances. Preferred Qualifications Prior experience in specialty area preferred.
