BRIEF SUMMARY OF THE JOB:

The Social Worker is responsible for providing case management, discharge planning, and counseling services to residents and families of the Nursing Home. In addition, the Social Worker maintains contact with community agencies and individuals who can contribute to a comprehensive plan of care when appropriate, and within the context of maintain confidentiality.  

DUTIES AND RESPONSIBILITIES

• Work cooperatively with colleagues and other staff.
• Assessment of the social, emotional and physical needs of the residents and their families.
• Exhibit knowledge of MDS 3.0
• Facilitate, participate and organize (as necessary) interdisciplinary care plan meetings.
• Maintain contact with community organizations to secure resources for residents.
• Timely documentation in medical charts.
• Participate in in-service education programs.
• Facilitate the grievance procedure upon resident or family request.
• Assures CCP Team in formulating an appropriate discharge plan for residents returning to the community.
• Assumes al other duties as assigned by the Nursing Home Administrator and Director of Social Work.
• Must have patience, tact, and cheerful disposition and enthusiasm, as well as willingness to handle residents based on whatever maturity level they are currently functioning.

PHYSICAL REQUIREMENTS   Must be able to move intermittently throughout the day.

• Must be able to speak English (language) in an understandable manner.
• Must be able to cope with the mental and emotional stress of the position.
• Must function independently, have personal integrity, have flexibility and the ability to work effectively with other personnel in social services and other departments.
• Must be good in mental health and emotional stability.

QUALIFICATIONS:

• preferred LMSW, MSW or BSW
• fluency in the Spanish Language required

The anticipated salary range listed is a good faith determination of the compensation that a successful applicant might receive at the time of this advertisement.

The pay range is subject to change at any time dependent on a variety of internal and external factors.

Actual compensation will be determined based on various factors including qualifications, education, experience, and location.

Job Type: Full-time

Salary: $60,000.00 per year
