The Advanced Practice Clinician is part of an interdisciplinary team providing comprehensive clinical assessments, acute and chronic disease management and advanced care planning. This role offers work life balance with no on-call requirements, no weekends and no holidays.Key ResponsibilitiesConduct comprehensive assessments for members in long-term care environmentsManage acute and chronic medical and behavioral health conditionsDocument conditions accurately, including ICD-10 codingCollaborate with an interdisciplinary care teamLead advanced illness and advanced care planning conversationsProvide education and counseling to patients and caregiversMaintain accurate clinical documentation in written and electronic systemsRequired QualificationsActive and unrestricted NP or PA license in the state of residence, or the ability to obtainGraduate of an accredited Nurse Practitioner (MSN or DNP) or Physician Assistant programActive national board certification NP: AANP or ANCC in FNP, ANP, GNP or AGACNPPA: NCCPA certificationActive DEA licensure or the ability to obtainAbility to lift a 30-pound clinical bag, navigate stairs and perform physical assessmentsReliable transportation for travel between facilitiesAbility to obtain a collaborative practice agreement if required in the stateFull COVID-19 vaccination (per federal and client requirements)Must pass a pre-employment drug screenPreferred QualificationsOne or more years of clinical practice experience, ideally in long-term careExperience working with patients who have complex medical, behavioral or functional needsBackground in geriatrics or chronic disease managementExperience with advanced illness discussionsProficiency with electronic medical documentationWork Environment & TravelTravel occurs during scheduled work hoursAPC will rotate between two long-term care facilities within Carroll and Galaxy City countiesNo on-call, weekend or holiday requirements
