Work Environment

The position is primarily onsite in the homes of our patients, allowing you to form meaningful relationships and provide focused therapy in familiar environments. Key Responsibilities

• Conduct detailed assessments to evaluate the physical condition of patients, reviewing their medical history and performing necessary evaluations to diagnose their needs.
• Articulate findings from evaluations to patients and their families, ensuring they understand the treatment plan and are involved in setting achievable health goals.
• Skillfully implement treatment protocols tailored to each patient's unique condition, adapting approaches based on their responses and the evolving healthcare standards.
• Collaborate with interdisciplinary healthcare teams to communicate significant changes in patient conditions and adjust care plans accordingly.
• Execute discharge planning and follow-up care with precision, documenting all care activities thoroughly to create a clear picture of the patient's journey, including initial assessments, ongoing progress notes, and discharge evaluations.
• Manage time and patient schedules efficiently, meeting productivity expectations while maintaining punctuality for meetings and patient appointments.
• Participate actively in hospital and community marketing efforts to promote our services and enhance the outreach of Scripps Physical Therapy Clinic.
• Contribute to the ongoing development of departmental goals, ensuring compliance with all state and federal regulations and accreditation standards.
• Pursue professional development through continuing education opportunities and serve as a mentor for students in clinical training.
• Support initiatives aimed at improving customer experience and access to care, resolving barriers actively through participation in Access NRH initiatives.

Patient Population / Client Focus

Our clinic serves a diverse patient population, including individuals recovering from surgery, those managing chronic diseases, and elderly patients needing rehabilitation services. Qualifications

• Doctorate Degree in Physical Therapy (DPT) is required.
• A valid DPT license in the state of New Jersey is mandatory for practice.

Preferred Experience

• While prior work experience is not a strict requirement, at least 2 years within a clinical setting is preferred.
• Experience in home health care is an advantage.

Compensation and Benefits
