Key Responsibilities

• Conduct comprehensive evaluations of patients, reviewing medical history, performing physical assessments, and providing documentation of findings.
• Communicate findings effectively to patients and their families, collaboratively setting achievable rehabilitation goals and ensuring understanding of the treatment process.
• Develop and implement individualized treatment plans based on thorough evaluation results, continually adapting protocols tailored to the patient's unique condition and progress.
• Work alongside other healthcare professionals to provide integrated care, being vigilant in recognizing changes in patient conditions that may require adjustments in treatment or referral to additional medical services.
• Oversee discharge planning and provide appropriate follow-up care recommendations, ensuring patients have access to resources for continued recovery post-discharge.
• Document patient interactions, treatment details, and outcomes accurately and in a timely manner to comply with healthcare regulations and maintain quality standards.

Patient Population / Client Focus

In this role, you will primarily serve adult patients with a range of conditions including post-surgical rehabilitation, chronic pain management, and neurological rehabilitation. Your focus will be on fostering functional independence and enhancing mobility in patients recovering from injuries or managing disabilities. You will work closely with patients of diverse backgrounds, addressing both their physical and emotional needs during the rehabilitation process.

Qualifications

• A Bachelor's degree in Physical Therapy from an accredited institution.
• Current state licensure or eligibility to obtain licensure in Virginia.
• Strong understanding of physical therapy principles and practices.
• Excellent communication and interpersonal skills, with the ability to relate well to patients and their families.

Preferred Experience

• Previous experience in an outpatient rehabilitation setting.
