Work Environment

This position is primarily onsite, requiring travel to patients' homes across Richmond, VA. You will experience a diverse range of home environments that present unique challenges and opportunities for patient interaction. You will work collaboratively with an interdisciplinary team, including other healthcare providers, to ensure seamless care and comprehensive treatment plans. Your efforts in home health care will provide you with the satisfaction of directly witnessing the positive outcomes of your work.

Key Responsibilities

• Conduct thorough evaluations of patients' physical conditions, including subjective history taking and objective assessments, to develop targeted treatment plans tailored to individual needs.
• Engage in clear and effective communication with patients and their families to explain evaluation findings, treatment goals, and the rehabilitation process.
• Implement evidence-based treatment protocols, employing therapeutic exercises, manual therapy techniques, and modalities as appropriate for each patient's condition.
• Monitor and document patient progress meticulously, adapting treatment plans as necessary based on real-time feedback and patient responsiveness.
• Collaborate with other healthcare professionals to ensure comprehensive patient care, including referrals for further medical or rehabilitation consultations when indicated.
• Manage and prioritize time effectively to fulfill patient caseloads while meeting productivity goals and healthcare regulations.
• Participate in discharge planning and follow-up care, ensuring a smooth transition for patients as they progress in their rehabilitation journey.
• Uphold the highest standards of patient care through compliance with all legal, regulatory, and accreditation requirements.
• Engage actively in continuing education opportunities to expand knowledge, skills, and therapeutic techniques to enhance patient outcomes.

Patient Population / Client Focus

In this role, you will treat diverse patient populations, including those recovering from orthopedic surgeries, managing chronic pain, celebrating functional improvements in activities of daily living, and elderly clients requiring mobility support. Each treatment plan will be individualized, with a significant emphasis placed on patient education and active participation in the rehabilitation process.

Qualifications

• A Bachelor's degree in Physical Therapy from an accredited institution.
• A valid license to practice physical therapy as per Virginia state regulations.
• CPR certification from the American Heart Association (BLS for Healthcare Providers) is required.
• Strong interpersonal skills and a dedication to providing compassionate healthcare.
• Proficiency in time management and the ability to work independently in a home care setting.

Preferred Experience

• Prior experience in home care physical therapy is preferred but not mandatory.
• Experience working with diverse patient populations is advantageous.

Compensation and Benefits
