Registered Nurse - Case Manager (Home Health)

Role Overview As a home care nurse, you will travel to patients' homes to deliver healthcare services, coordinating and providing both direct and indirect patient care using the nursing process. You will assess patient needs, develop care plans, monitor progress, and educate patients and families, ensuring high-quality care with minimal supervision. You will act as a team coordinator, supervise home health aides, and perform complex wound care, IV management, and assessments such as OASIS. Responsibilities Include Understanding Medicare benefits, documentation standards, and regulatory requirements Performing case management activities and maintaining accurate records Communicating with physicians, caregivers, and other healthcare team members Leading and supervising care activities, including aide assignments and home visits Conducting assessments, wound care, central line management, and IV procedures Ensuring compliance with federal and state guidelines Qualifications And Skills Valid nursing license (RN) and required certifications (BLS, auto insurance, driver’s license) Minimum one year of nursing experience Proficient interpersonal, time management, and communication skills Ability to connect with patients and deliver compassionate care #J-18808-Ljbffr DayToDay Health
