Home Health Account Executive

Who We're Looking For

• 2+ years of home health sales experience
• Established book of business and strong referral relationships
• Proven track record of meeting or exceeding sales targets
• Communicate, present, and build relationships with confidence
• Work independently, stay organized, and manage their time well
• Have reliable transportation and a valid driver's license

What You'll Do

As the face of Preferred Care in your territory, you will:

• Grow and deepen referral relationships with physicians, hospitals, discharge planners, case managers, social workers, skilled nursing facilities, and assisted living communities
• Drive market share through strategic territory planning and business development
• Deliver polished presentations that showcase our services and clinical programs
• Serve as the go-to liaison between referral sources, patients, physicians, and our internal team
• Pre-screen referrals for home health eligibility and manage physician orders through to signed completion
• Stay in regular contact with referral partners to keep patient care on track
• Represent Preferred Care at networking events, healthcare association meetings, and community outreach
• Maintain accurate activity reports and documentation
• Stay current on Medicare guidelines and home health regulations
• Partner closely with clinical and operations teams for smooth patient transitions
