Job Requirements

• Minimum 2 years of social work experience required
• Recent acute care social work experience required
• Master of Social Work (MSW) from a CSWE-accredited school of social work
• Active Alaska LMSW license required; candidates eligible for a temporary Alaska LMSW license may apply
• Ability to work collaboratively with interdisciplinary care teams
• Strong assessment, discharge planning, crisis intervention, and care coordination skills
• Experience supporting patients across inpatient, emergency care, or hospital-based settings preferred

Responsibilities

• Complete psychosocial assessments and provide emotional support to patients and families
• Develop safe, ethical, and person-centered discharge plans in alignment with policy and regulatory standards
• Provide brief clinical interventions, resource navigation, and coordination of care
• Screen for suicidality, support safety planning, and complete required reports based on state guidelines
• Collaborate closely with RN Case Managers and the interdisciplinary care team
• Support patient populations across adult, pediatric, emergency, and specialty care areas as needed within scope
• Coordinate with community agencies and outside entities to promote continuity of care
• Float to like units when needed to support facility operations
