RESIDENTIAL HABILITATION COORDINATOR

JOB SUMMARY

The Residential Habilitation Coordinator is responsible for the development, implementation, and ongoing evaluation of individualized habilitation plans for people with intellectual and developmental disabilities residing in Venture Together residences. This role ensures that all services are delivered in accordance with Person-Centered Planning principles, regulatory requirements, and best practices.
The Residential Habilitation Coordinator provides oversight of Staff Action Plans and is accountable for ensuring the Direct Support Professionals (DSPs) are effectively trained and supports consistent implementation of plans as written. The coordinator monitors individual progress toward outcomes, ensures health and safety needs are met, and drives continuous improvement in service delivery through data-informed decision-making. The coordinator supervises Residential Habilitation Specialists. Working collaboratively with the Residential Director and interdisciplinary team members, the Residential Habilitation Coordinator plays a key role in aligning daily support
with each individual’s Life Plan and ensuring compliance with all applicable regulatory and agency standards.

The Residential Habilitation Coordinator reports to the Director of Residential Services.

QUALIFICATIONS

• Bachelor’s degree in human services, psychology, social work, education, or related field.
• Minimum of 3 years’ experience working with individuals with intellectual and developmental disabilities combined with at least 2-4 years in a supervisory or management role.
• Demonstrated experience developing and implementing habilitation or service plans independently.
• Knowledge of Person-Centered Planning principles and practices; OPWDD regulations and CMS HCBS
  requirements.
• Strong written and verbal communication skills, including documentation proficiency.
• Ability to analyze data and translate findings into actionable service improvements.
• Valid driver’s license with at least 2 years of driving experience meeting agency insurance requirements; Use of own vehicle for regular travel between residential sites is required.
• Experience in a residential services setting preferred.
• Supervisory or leadership experience preferred.

Salary range: $60,000 – $65,000 per year

Location: Rockland County, NY

Hours: 9:00am – 5:00pm 

Days: Monday – Friday 

Full Time: 40 hours per week
