This full-time position involves engaging with clients in a hospice care environment and addressing severe emotional disorders and major mental illnesses. Work Environment

This role is primarily remote, allowing flexibility while ensuring that our social workers can connect with clients effectively through telehealth technologies and home visits when necessary. Key Responsibilities

The LCSW will engage in a variety of crucial tasks, which include but are not limited to:

• Conducting comprehensive assessments and providing individualized treatment plans for clients dealing with significant mental health issues.
• Providing direct support through individual, group, and family therapy sessions, adapting approaches to meet the unique needs of each client.
• Collaborating with multidisciplinary teams to facilitate integrated care, ensuring clients are linked with necessary services and resources.
• Actively participating in crisis intervention efforts, responding promptly to client needs to ensure safety and well-being.
• Maintaining accurate case documentation and monitoring client progress, including timely completion of reports for insurance and regulatory purposes.
• Facilitating workshops or support groups to empower clients and their families, addressing mental health awareness and coping strategies.
• Participating in committees dedicated to enhancing service delivery and quality improvements within the organization.

Patient Population / Client Focus

The client population will primarily consist of individuals facing chronic illnesses and severe psychological distress typical within hospice settings. The LCSW will work closely with patients, families, and caregivers, understanding their emotional and psychological needs during sensitive end-of-life processes. This role demands empathy, acute listening skills, and a commitment to holistic care, ensuring that every client receives compassionate support tailored to their circumstances.

Qualifications

The ideal candidate will possess the following qualifications:

• Must hold an active Licensed Clinical Social Worker (LCSW) credential in New Jersey.
• A Bachelor's degree in social work or related field is required, with a strong preference for candidates holding a Master's degree.
• Proven ability to establish trusted relationships with clients from diverse backgrounds and an understanding of cultural competencies.
• Exceptional verbal and written communication skills necessary for effective client interaction and documentation.

Preferred Experience

While a minimum of 2 years of clinical experience is required, candidates with a background in hospice care or medical social work and proficiency in mental health assessments will be prioritized. Experience implementing evidence-based treatment practices is also highly valued.

Compensation and Benefits

This position offers a competitive salary range of USD 70,000 to 85,000 annually, commensurate with experience.
