3. ABOUT THIS POSITION

As Declare Wellness Center continues to grow, we are expanding our clinical team and seeking a Functional Medicine Provider (NP, PA, MD, or DO) to take full ownership of patient care alongside our founder. This is a meaningful, high-impact role at the center of everything we do - managing patient consultations, hormone and lab oversight, and individualized care planning across our active wellness programs and membership patient panel.

From day one, you will work directly with Dr. Craig Cohen DC, FNP, learning the Declare model and progressively building your patient relationships. Collaborative clinical relationships and physician support are in place, and in-house training in functional medicine, BHRT, genetics, and peptide therapy is available to support your development. In your first 90 days, success looks like completing your clinical orientation, building strong patient rapport, and confidently managing follow-up visits and lab reviews independently.

This role is ideal for a provider who enjoys building long-term patient relationships, thrives in a collaborative clinical environment, and is excited to grow within a rapidly expanding functional medicine practice.

We are open to both full-time and part-time providers, and we welcome candidates who are willing to relocate to the Murrells Inlet, SC area.

4. KEY RESPONSIBILITIES

Patient Care & Clinical Management

● Conduct initial consultations, review-of-findings appointments, and follow-up visits for active wellness program members and VIP hormone patients.

● Manage BHRT and hormone optimization care, including developing and adjusting individualized treatment plans.

● Collaborate with the health coach to align medical and lifestyle interventions and ensure timely patient follow-up.

● Facilitate patient workshops and educational sessions to increase engagement and strengthen clinical touchpoints between visits.

Lab Management & Prescriptive Oversight

● Review and interpret weekly lab results, order follow-up testing, and adjust treatment plans accordingly.

● Prescribe and manage hormone therapies, peptides, and related treatments within Declare’s collaborative physician arrangement.

● Maintain complete and accurate clinical documentation within required EMR timelines.

Practice Operations & Team Collaboration 

● Participate in daily morning huddles and weekly team training sessions.

● Attend regular clinical check-ins and case reviews with practice leadership.

● Utilize Declare’s systems - including EMR platforms, Google Drive, and shared templates — to maintain consistency and operational efficiency.

Patient Engagement & Retention 

● Track patient progress across active programs and VIP memberships, proactively identifying patients who may benefit from additional support or program transitions.

● Communicate with patients between scheduled visits to reinforce treatment adherence and continuity of care.

● Represent the Declare standard of care in every patient interaction - engaged, knowledgeable, and genuinely invested in patient outcomes.

5. SKILLS & QUALIFICATIONS

Required

•      Active NP, PA, MD, or DO license with prescriptive authority

•      SC licensure or active eligibility to obtain prior to start date

•      Minimum 2 years post-licensure clinical experience

•      Genuine interest in and exposure to functional medicine and/or hormone therapy

•      Strong patient communication and interpersonal skills

•      Comfort with digital tools - EMR systems, lab ordering platforms, and Google Suite

Preferred (Nice to Have)

•      Experience with BHRT or hormone optimization

•      Functional medicine certification (IFM, A4M, or equivalent)

•      Experience with peptide therapy or genetics-based protocols

•      Background in a cash-pay or membership-based practice model

•      Personal engagement in health optimization, fitness, or biohacking

7. COMPENSATION & BENEFITS

Salary: Starting at $95-105k per year, based on experience

Bonus Structure: Performance-based bonus; eligible after the 90-day introductory period

Schedule: Full-Time or Part-Time available | Full-Time: Monday - Thursday, 8:00 AM - 4:30 PM | Fridays required during initial onboarding and training | No weekends or holidays | Part-Time hours and days discussed based on candidate availability and practice needs

A note on benefits: Declare Wellness Center is a small, independent practice, which means we do not currently offer employer-sponsored health insurance.

We want to be transparent about this upfront so you can make an informed decision before investing time in our hiring process.

Our team members typically obtain coverage through a spouse or partner, the ACA marketplace, or a health-sharing plan, and we’re happy to discuss these options during the interview process.

By the six-month mark, success looks like a full patient panel, efficient lab review days, strong collaboration with the health coach, and patients who feel genuinely supported throughout their care journey.
