Physical Therapist ( PT ), Home Health - $10,000 Sign on Bonus Available

Overview Now Hiring: Registered Physical Therapists (Full-Time or PRN) We are seeking a Registered Physical Therapist to provide skilled, intermittent physical therapy in patients\' homes according to physician orders and individualized care plans. You will work under the supervision of the Director of Clinical Services, Nursing Supervisor, or Rehab Director. This role supports delivering high-quality, 1:1 care while minimizing documentation. Responsibilities Understands and adheres to established policies and procedures. Provides physician-prescribed physical therapy. Improves or minimizes residual physical disabilities of the patient. Returns the individual to an optimum and productive level within the patient’s capabilities. Participates with all other health care personnel in patient care planning. Directs and supervises personnel as required. Takes initial history and performs initial evaluation. Performs all skilled procedures as ordered by physician. Consults with physicians regarding changes in treatment. Writes reports to physicians regarding patient progress. Instructs patients and family/significant others in home programs and activities of daily living. Participates in in-service programs and presents in-service programs as assigned. Participates in performance improvement activities as assigned. Attends all patient care conferences as scheduled. Prepares clinical and progress notes. Assists physician with evaluating level of function. Helps develop and revise the plan of care as necessary. Consults with family and Agency personnel. Completes and submits OASIS assessments, reassessments, transfers, resumptions of care, discharges, and significant changes in condition within Agency-defined time frames. Appropriately utilizes ICD-9 codes. Qualifications Graduate from a physical therapy curriculum approved by the American Physical Therapy Association, or other accredited body, with current license in the practicing state(s). Two (2) years experience, preferred. Acceptance of philosophy and goals of the sponsoring Agency. Ability to exercise initiative and independent judgment. Physical Requirements Ability to participate in physical activity. Ability to work for extended periods while standing and being involved in physical activity. Heavy lifting; ability to do extensive bending, lifting and standing on a regular basis. Working Environment Works indoors in the Agency office and patient homes and travels to/from patient homes.
