Clinical Admissions Liaison | In-Person Full-Time

Bane Care Management is seeking an experienced and motivated Clinical Admissions Liaison to join our admissions team in Braintree, MA. The Clinical Admissions Liaison is responsible for assessing challenging and clinically complex patients prior to admission to one of our skilled nursing centers, to ensure all clinical, fiscal and regulatory criteria are met.

As a longstanding, premiere post-acute care provider, BaneCare Management has the privilege of helping families and patients with the continuity of care as they transition from acute hospital providers to a post-acute rehab or long-term care setting. The Clinical Admissions Liaison will work with new and existing referral sources and will be responsible for maintaining admission and census levels. In addition, the Clinical Admissions Liaison will be tasked with growing relationships with hospital providers and our skilled nursing centers members on a daily basis.

Position Details

· Clinical Admissions Liaison is a full-time in-person position

· Salaried position with full benefits

· Position is primarily in-house. Must be able to travel to various referral sources and skilled nursing centers on occasion

· Required: valid driver’s license, dependable transportation and auto-insurance policy

Strongly Recommended Skills & Certifications:

· Clinical degree, ie RN/LPN/SW/Rehab. Non-clinical degree considered with relevant experience.

· Prior experience as SNF clinical liaison preferred

· Excellent organizational skills with effective time management.

· Effective interpersonal skills.

· Strong clinical assessment abilities with capability to make independent admission decisions

· Demonstrate integrity and professionalism in all interactions and adhere to all HIPAA rules for patient privacy and security.

· Familiarity with regulatory standards and compliance requirements.

· Excellent oral and written communication skills

· Proficiency in computer skills including Microsoft Office, hospital referral platforms and electronic medical records.

· Experience with case management, utilization review, insurance companies, and local hospitals preferred
