About the Role
As a Clinical Liaison, you will play a vital role in ensuring seamless communication between our healthcare facility and referring hospitals, healthcare providers, and other stakeholders. You will be responsible for fostering strong relationships with our partners, coordinating patient transfers, and providing high-quality patient care. Your expertise will enable us to deliver exceptional patient outcomes while promoting efficient and cost-effective care delivery.
Responsibilities
• Develop and maintain strong relationships with referring hospitals, healthcare providers, and other stakeholders to ensure effective communication and coordination of patient care
• Conduct patient assessments and develop personalized care plans to ensure optimal outcomes
• Coordinate patient transfers, including arranging transportation and communicating with patients, families, and healthcare providers
• Collaborate with interdisciplinary teams to ensure comprehensive care and address any issues or concerns
• Participate in quality improvement initiatives to enhance patient care and satisfaction
• Maintain accurate and timely records of patient interactions and communications
