RN Case Manager: Pre-Admission & Care Coordination

Henry Mayo Newhall Hospital in Valencia, CA is seeking an RN Case Manager-Pre Access to screen, coordinate, and facilitate admission reviews for acute patients, ensuring adherence to medical guidelines in collaboration with credentialed medical staff. You will work with interdisciplinary teams to coordinate transfers and communicate admissions with the Director of Case Management and Physician Advisor as needed.
