RN Case Manager - Acute Care, Day Shift

Working Nurse

Henry Mayo Newhall Hospital in Santa Clarita, CA is seeking an experienced RN Case Manager to coordinate care across the continuum, perform utilization review, and connect patients, families, and nursing units with social work and post-hospital planning. The role requires CA RN license, BLS, and case management certification preferred; two years in acute care or five years nursing experience preferred.
