Work Environment

This position is based onsite at an assisted living facility in Houston, Texas, where you will work alongside a talented team of healthcare providers. The environment fosters collaboration and a strong commitment to patient-centered care. You will work approximately 40 hours a week, with a typical schedule from 9:00 a.m. to 6:00 p.m. and occasional evening clinics until 8:00 p.m. Participation in weekend community outreach events may also be required, enhancing your clinical practice and community engagement.

Key Responsibilities

• Perform comprehensive health assessments for residents, including obtaining medical histories, conducting physical examinations, and identifying health risks.
• Diagnose and manage both acute and chronic health conditions, developing individualized care plans focused on preventive measures and health promotion strategies.
• Work collaboratively with community organizations to design and execute health education programs tailored to the needs of various populations.
• Champion culturally competent care by advocating for patients’ rights, ensuring they receive appropriate services, and addressing barriers to healthcare access.
• Oversee the coordination of referrals to specialists and follow-up care, ensuring seamless continuity of care.
• Document patient interactions comprehensively and accurately in electronic health records, adhering to regulatory and insurance standards.
• Engage in community events and health fairs, providing screenings and health information to educate attendees.
• Stay informed about emerging health trends, legislation, and best practices affecting community health, implementing evidence-based care approaches.

Patient Population / Client Focus

The client base primarily comprises residents of an assisted living facility, many of whom belong to diverse ethnic backgrounds and face socio-economic challenges. You will engage with individuals who often have complex health needs, requiring a tailored approach to care that respects cultural sensitivities and promotes health equity. Your work will directly impact the health outcomes of these vulnerable populations, enabling them to lead healthier lives.

Qualifications

• Active Nurse Practitioner certification and a valid state nursing license.
• Fluency in Spanish, enabling effective communication with a significant portion of our patient population.
• A Master’s degree in nursing from an accredited institution.
• A minimum of three years of clinical experience, particularly in community or public health settings, is essential.
• Demonstrated ability to forge strong patient relationships through exceptional interpersonal and communication skills.
• A commitment to social justice and health equity, along with a passion for advocating for underserved communities.

Preferred Experience

• Experience working in assisted living facilities or similar environments.
• Previous involvement in community outreach programs and health education initiatives.
• Familiarity with electronic health record systems and patient management software.

Compensation and Benefits

We offer a competitive salary range of $120,000 to $130,000 annually, commensurate with experience and qualifications.
